Dear Community Nurse
Re: Ms Monica Osburn, aged 69
The purpose of this letter is to refer the aforementioned patient, who was diagnosed with hypertension and depression, requires regular monitoring and ongoing care.
She has had hypertension for the last 10 years, as well as depression from last 2 years. Has been prescribed with medication such as: anti-hypertensive and anti-depressant.
Used to live alone however, on request of only daughter, moving in Woodville near to daughter's house.
Habit of excessive alcohol consumption has been observed over the past years, and surge can be seen whenever feel anxious. Therefore, frequent supervision will be require by a community nurse to assure the intake of alcohol and medication.
Visit to a medical practitioner has been recommended after moving. As well as it will be highly appreciate to supervise the dietary intake, alcohol intake, likewise medication regime.
Motivate to be a part of social activities such as: join elderly citizens' clubs' .Likewise voluntary groups to ensure social interaction.
