Re: Mr Gerald Baker, aged 79
Mr Baker, who underwent a left total hip replacement surgery, requires ongoing care and management at your facility, following discharge back today.
Mr Baker was recommended on aspirin 100 mg daily postoperative in the morning. In addition, ramipril 5 mg daily in the morning and co-codamol daily four times as required for hypertension and pain respectively. During recovery postoperative disorientation occurs with the effect of Anesthesia, requires continuous monitoring. During hospitalization, three units of packed red blood cells transfused postoperatively.
In the recovery period, stipulated programs must be performed; regular dressing changes must be performed; dietary regimen must be enforced; exercise regimen should be implemented in which stretching and strengthening exercises must be continued. Weight-bearing should be performed with the help of wheelie-walker. Signs of anemia should be observed continuously.
Follow-up after two weeks should be performed in the City Hospital Clinic for the stapler removal and the blood tests.
Based on the aforementioned circumstances, regular supervision and appropriate management would be extremely beneficial to improve Mr Baker condition. Incase, additional information is required, kindly do not hesitate to contact me.
