Re: Ms Monica Osburn,aged 69

I am referring Ms Monica Osburn, Who was diagnosed with hypertension and depression, for regular monitoring and ongoing care.

Ms Osburn has had hypertension and depression for the last ten and two years respectively. For which anti-hypertensive and anti-depressant drugs have been taken by her. Problem related to self-administration of medications have been reported. Ms Osburn has history of excessive alcohol consumption which increased drastically while upset was noted.

Therefore, regular assessment by medical specialist is advised. Daily home visit by a community health nurse for medications compliance and dietary regiman is required. Encouragement regarding participation in recreational and social activities would be appreciated.
