Dear Nurse,


Re : Mr Phillip Satchell

Age: 73 years


I am writing to request a home visit for Mr.Phillip Satchell who has been diagnosed with type- II diabetes with chronic right and left leg ulcers.He requires continuing care and further management from your service. He is being transfered today.


Mr Satchell is a widower and lives alone. He has two sons and has small pension . He is non-complaince with his oral diabetic agents and a diabetic diet . He has frequent relapses of multiple sclerosis(Ms). He will move to your facility to increase access for MS treatment.


Mr Satchell has pus forming staphylococcus aureus infection in the leg ulcer. He was prescribed intravenous antibiotics twice daily and metformin three times per day. He has been recommended to perform daily wound dressings to both the legs,which needs to be continued.He was monitored for signs of MS relapse,blood sugar levels and medication compliance. He was educated regarding diabetes.


It would be appreciated if you could switch to oral antibiotic for Mr Satchell. It is essential to continue the same diabetic medications. Kindly refer him to Prince of Wales Diabetic Clinic and neurologist for medication review and MS follow-up care. Please support him to link with community services to increase coping and social network.
