Re:Ms Mary Bell

Age:66 years


I am writing to refer Ms Mary Bell who has small infected left foot ulcer. She requires overall life-style plan from your service. She is being discharged today.


Ms Bell is single and lives alone in her own four-bedroom home. She was retired from managerial position and has small pension. Her alcohol intake has increased and dietary quality has decreased since her retirement. She has Non-Insulin dependent diabetes mellitus for the past fifteen years,for which she is on glibenclamide,5 mg daily and metformin, 850 mg thrice a day.

She has periodic problems with self-administration of hpoglycaemic medication.


Ms Bell was admitted to hospital with unstable blood sugar level.she was prescribed amoxicillin,875/125 mg twice a day. She was recommended to perform daily wound dressing. She was monitored regarding wound margins,signs of complications.


It would be appreciated if you could monitor medication compliance,alcohol intake,diet and blood sugar levels for Ms Bell. It is essential for her to review antibiotic theory at the completion of current course. Kindly encourage her to perform moderate exercise programme and establishment of social and income-producing activites . It is vital for her to involvement of community socical worker service.
