Re: Ms Sheila Cartwright, aged 69
I am writing to update you regarding aforementioned patient, who is to be discharged today following her admission on 14 April 2017, due to unstable diabetes mellitus and ulcer on left foot.
Has non- insulin dependent diabetes mellitus from past 9 years, therefore sometime she found it straneous to administer medication by her self. Moreover has minimal social interaction, as she lives alone. Consequently she has a habit of excessive alcohol consumption. On the contrary, standard of diet become inadequate, becuase of meagre income.
She has commenced on some medication such as: Glimel, 5mg, daily; Diabex, 800mg, 3 times a day, and Augmentin Duo Forte, 125mg, orally, twice a day.
In addition to these glibenclamide should be carry on
Do arrange moderate exercise session, to prevent further ulceration. Furthermore emphasize on task, those are ausphicious in generating income. In addition motivate the pt to be a part of social event. Afterwards focus on healthy dietary habits too.
Even though pt is quite stable, but she is single and quite old so still requires supervision of a community nurse.
Regularly observe for any complication regarding medication and watch on levels of blood sugar and alcohol intake.
