Dear Nurse,
Re: Mr Alfred Billy, aged 52
Mr Alfred Billy, who underwent basal cell carcinoma removal surgery, requires further assessment and appropriate management following his discharge today.
On 21 March 2010, Mr Billy was admitted to the hospital. Skin biopsy revealed basal cell carcinoma on the neck. Following the biopsy mild wound infection was developed may be due to suture reactions or incomplete excision. Post-operatively, he has recuperated well and his pain was managed with Panadeine forte 500mg.
Mr Billy is a truck driver. He smokes regularly and consumes alcohol.
It would be greatly appreciated if you could perform daily observations for Mr Billy. Please monitor his medication compliance. Continuation of medication is required for another week.
In case any additional information is required, please do not hesitate to contact me.
Yours sincerely,
Charge Nurse
