Re: Ms Betsie Anderson, aged 89
Ms Betsie Anderson, who was diagnosed with urinary tract infection, requires further assessment and appropriate management following her discharged today.
On 16 February 2020, Ms Anderson presented to the Norfolk and Norwich University Hospital with low grade fever, increased confusion and somnolence. Investigations revealed diagnosis and it is being treated with IV ciprofloxacin. Codeine allergy was reported. Recently urinary incontinence was developed and weight loss of six pounds in one week time was reported.
It would be greatly appreciated if you could continue to assess the vital signs daily. A follow up visits with the general practitioner, Dr Alex Rose, in 2 weeks’ time has been scheduled. Urine analysis and laboratory tests will have to be repeated in 2 weeks’ time. Urine and blood culture reports have to be collected which are pending. Signs of dehydration and orthostatic blood pressure have to be observed closely. Continuous monitoring of medication compliance is required. The discharge medications are: levothyroxine 200 mcg, Colace 100 mg Morning, ciprofloxacin 250 mg every 12 hours orally for 14 days and tablet multivitamin with calcium at night. She also has to take protonic 75 mg daily and Aricept 10 mg every night is prescribed.
