Dear Dr. Robinson
Re:Mr. Edwin Bruker
Thank you for accepting Mr. Bruker, who has been suffering from stroke associated depression requires your expert advice and management for the ongoing behavioural change.
During the recovery process, Mr. Bruker’s progress was doing excellent, however some behavioural alteration was noticed. In addition, social and daily living activities were totally neglected. A significant weight loss of almost 6kg was observed. Soon after the changes detected,a cognitive behavioural therapy and daily counselling was commenced by the community health nurse.
Hence, the ongoing symptoms have been severely worsened, increasing more signs of paranoid behaviour and delusions. The behavioural change have been so remarkable that Mr.Bruker attempted for su ode, fortunately saved by the caregivers.
Based on the aforementioned circumstances, your through evaluation and superior management for the ongoing behavioural change would be profoundly beneficial for Mr.Edwin Bruker’s health condition. In case, you require any additional information please do not hesitate to contact me.
