Re: Mrs.Rachel Andrewson,aged 60


Mrs.Rachel Andrewson,who was diagnosed with renal failure and is on hemodialysis,is being discharged back into your facility today.She requires constant monitoring and appropriate assistance.


During hospitalisation,vitals were unremarkable.An emergency hemodialysis was arranged due to persistent underlying symptoms.Additionally,denied to take medications as well as food and was observed to be disoriented.Due to improper dietary habits,impact was noticed on her weight.An expert opinion was taken from the dietician.Cessation of alcohol and smoking was highly suggested.


Fluid restriction 1lit/day along with dietary restrictions was advised by the nephrologist.A weekly hemodialysis was scheduled.As per examination,she showed good prognosis towards the treatment.Range of motion exercises was initiated and explained the importance of its continuity.Review after 1 week for the further assessment in the OPD


Based on the aforementioned circumstances,regular monitoring and uninterrupted assistance would be profoundly beneficial for Mrs.Rachel’s health condition.In case,you require any additional information, please do not hesitate to contact me.
