Dear Head Nurse,


Re: Mr Robert Jai, aged 70


Mr Jay, who has been recuperating from appendectomy, requires your support and ongoing care following his discharge today.


On 10th May, Mr Jai presented with severe abdominal pain. On examination, ultrasonography revealed an inflammation of Appendix. Along with that, the levels of CBC, WBC, and RBS were also raised. Consequently, a surgery was performed on the next day. Post operatively, Injection diclofenac and Taxim one gram were commenced.


After two days, wound oozing with inflammation was noticed which got rectified by changing dressing regularly and shifting him to Augmentin. Presently, wound is healing but continuous dressing is still required. Although Mr Jai has started mobilisation, physiotherapy and assistance with daily living activities would be beneficial.


Pertaining to his social background, Retired postman Mr Jai is a widower and lives alone. He has been smoking for the past 30 years. Also, he has had hypertension and diabetes mellitus since ten years.


In view of the above, you are requested to provide continuous monitoring for ensuring medication compliance and blood glucose as well. In addition, provision of soft and liquid diet would be highly appreciated. Please note, Mr Jai is allergic to panadine Forte. His follow-up appointment has been scheduled after one week.
