Re: Mrs Silvia Rule, aged 69
Mrs Rule, who has bacterial chest infection and alzheimer, is being transferred into your facility following her discharge today. I am updating the current health information along with the required follow-up care for your perusal.
On 10 June, Mrs Rule was brought to the emergency department with the symptoms of wheezing, shortness of breath and cough. In addition, fever and fatigue were also noticed. Following this, x- ray, computed tomography and complete blood test which were perfomed, revealed the exacerbation of previous respiratory infection, for which antibiotic treatment was done.
Therefore, oxygen therapy and the and the injection amoxicillin, 500mg for 3 day have been administered. The Latter of which has to be continued for 1 week. Despite, good overall progress, restlessness, agressive behaviour and reactants for ward admission have been observed. Subsequently, instant discharge was advised with the family members. Monitoring has been suggested regarding not only medication compliance but also for the signs of hyperthermia and breathing problems.
Based on the aforementioned circumstances, ongoing care and monitoring of Mrs Rule's condition would be profoundly beneficial.
