Dear Ms. Adams,
RE: Mr. Martin Wilson
I a writing to refer Mr Wilson who is recuperating from the complications after suicide attempt. He requires urgent attention for the follow up care
Mr. Wilson was admitted to the hospital on 13th of this month due to overdose Mogodol. During his hospital stay, he was weak and depressed. He has been treated with anti- depressants. He also diagnosed to have type II diabetes. He is able to use wheelchair from 20t of this month.
Mr Wilson who is a retired bookkeeper, lives with his wife and son. He drinks alcohol daily and addicted to gambling. Depression developed due to gambling addiction and he lost his for that reason.
It would be greatly appreciated if you could monitor the follow up care for Mr. Wilson. He requires gambling addiction therapy and it is important to encourage him to join gambling addiction action group at Elizabet community center. He might require the wheelchair for another one week and frame can be used for walking. Importantly, please motivate him to involve social sporting activities such as lawn bowls.
Please do not hesitate to contact me if you have any further queries
Yours sincerely,
