Re: Ms Monica Osburn, aged 69
Ms Osburn who will be relocating to Woodville requires unceasing supervision and monitoring. She has been diagnosed with hypertension and depression.
Ms Osburn is having hypertension and depression for the past 10 and 2 years respectively. Consequently, treated with ant- hypertensives and anti- depressants. Additionally, non-compliance with self-administration of medication was reported.
Ms Osburn is a divorcee and resides alone in rented accommodation. Presently, moving along with her daughter. The aged pension has been the source of income. Consumption of alcohol above the normal limits when anxious was documented.
Upon Ms Osburn's transfer, the establishment of contact with the general practitioner would be greatly appreciated. Restriction of alcohol intake along with compliance with the medication and dietary regime required to be monitored. Pertaining to family and social activities involvement in elderly citizens clubs and voluntary groups should be ascertained.
