Dear Nurse,

I am referring Ms Elma Edith, who was diagnosed with the early stage of cirrhosis, for ongoing care and further assistance at your facility, following the discharge today.

Ms Edith was presented to the hospital on 17-10- 2010 with the complaints of nausea, vomiting and abdominal pain in the right upper quadrant. She also experienced a weight loss of 3 kg. As per examination, she showed good prognosis towards the treatment of 20 days. Therefore the surgery is not necessary, requires to follow post medical care and treatment.

Ms Edith known case of hypertension which was initially high, are currently stabilized. She lives with her disabled husband.

Following discharge, Ms Edith's husband requested to provide a nurse for personal care . Kindly ensure that she receives post medical care and treatment are highly recommended.
