Re: Mr Bill O' Riley, DOB: 12 January 1956
Dear Sir / Ma'am,
I am writing this letter on the behalf of Mr O' Riley who was diagnosed with obstructive coronary artery disease, underwent a surgery for bypass graft, requires health education on low fat diet, recipes of some healthy food and appropriate management at your facility, following the discharge today.
Mr O' Riley a fencing contractor, lives alone, an allergic to nuts. He is an alcoholic as well as a chain smoker. He usually takes sausages, deep fried chips, eggs in his diet was noticed. Ideal body weight of 99 kg was reported, whereas the height was 170 cm considerably overweight due to unbalanced diet. As per examination, he showed good prognosis towards the treatment of post surgery.
Following discharge, gradual reduction of alcohol consumption is advised. Additionally, dietary regimen and smoking cessation are suggested. Apart from that, the healing of the wound is progressing, needs social worker to pay a daily visit to his house. Moderate exercise programme should be encouraged. A review with local general practitioner and physiotherapist after 6 days is scheduled.
