Re: Mr Jim Middleton, aged 84

Dear Sir/Ma'am,

I am referring Mr Jim Middleton who has undergone a surgery for left inguinal hernia, for ongoing care and further assistance at your facility, following the discharge today.

Past medical history revealed that he has had hypertension since 1998 for which he is taking tablet Atacand 4 mg once a day.

Mr Middleton, second world war veteran, lives with his wife, are independent, applied for aged pension. Performing household activities is not possible thing. Additionally, he requires to pay a home visit to their house.

Following discharge, lifting heavy objects and driving are prohibited. Light exercise is advised whereas tablet panadol thrice a day was prescribed. A review with the surgeon after 2 days is scheduled.
