Re: Ms. Mary Bell, aged 66


Ms. Bell who was diagnosed with unstable diabetes mellitus and small infected left foot ulcer, requires continuity of care following her discharge today.


Ms. Bell has had non insulin-dependent diabetes mellitus for fiften years. Upon admission hypoglycemic agents and antibiotics were prescribed. A review at the completion of the glibenclamide and augnmentin is scheduled. Apparently healingprocess of the ulcer as well as the signs of infertuin heas to be noticed. Daily saline dressing is advised.


After te discharge Ms. Bell will be alone in her house, hence no outside interests and no close relationship were reported. There fore,edication complaince dietary habits and alcohol intake has to observed. Socio-economic, acitivity activity along with moderate exercise programme has to be encouraged. Blood sugar level also monitored regularly.


Based on the aforementioned circumstances ongoing supervision would be greatly beneficial. Incase you require any additional information, please do not hesitate to contact me.
