Re: Ms. Sonia Calleja, aged 85
I am referring Ms. Sonia Calleja, who was diagnosed with right inguinal homia requires your assistance for a transition to an aged care facility for regular monitoring and continuing care following her discharge.
On 28 August, Ms. Calleja presented complaining sudden onset of pain, nowsea, constipationand bloated abdomen and diagnosed with intestinal obstruction. She underwent open surgery to repair right inguinal hermia. Paracetamol was commenced for pain management. Post-operatively, there was good prognosis and vital signs were stable. She was referred to gersatric rehabilitation anf assessment unit. Provided 4 wheel frame for walking, as she is frail and more to fall and also noticed confusion and frequent faecal incantinence. She was assessed by aged care assessment team and recommendation for a permanent 24 hours nursing care.
Ms. Calleja lives alone in an independent living unit. She has had asteroarthritis, managed by iron supplements and vitamin D.
Based on the above mentioned circumstances Ms. Calleja is unhappy and anxious living in independent living unit post operatively, her daughter is trustrated as she is unwell and unable to support her mother. Arranging for a transition to aged care to provide support and continuing care would be profoundly beneficial.
