Dear Midwife

Re: Alicia Nesbitt, aged 6 days

I am referring Alicia Nesbitt, who has been diagnosed wjth low birth weight and opioid dependence requires follow up care and management following her discharge today.

Alicia was born 6 days agl in the hospital to a sex worker mother, Ms. Gemma, who has been heraun dependent. At birth her weight was 2 kgs. She has been bottle feeding erotically and showed poor appetite. Ms. Gemma has poor bonding with Alicia, after requires to prompt regarding Alicia's care. Drug and alcohol team involved in managing addiction issue. Both the mother and Alicia completed heroin withdrawl therapy succesfully. After the social services have been involved. She is authorized to live with her maternal grandmother. However, she weighted 2.3kgs on discharge.

Alicia's mother is 20 years old, heroin dependent since 2 yearsand estranged from Alicia's father due to domestic violence during pregnancy. Her grandmother is supportive.

Based on the above mentioned circumstances: daily visits until Alicia weight and fonding stable is necessary; education provided to the mother as well as grandmother regarding Alicia's care, coping and psychosocial status of Alicias mother is advised. Communication with drug and alcoholteam to provide integrated support for Ms. Gemma to reduce me risk of heroin use is strongly recommended.

Ensuring safe environment and to update the social worker if there are onlg risks would be profoundly beneficial to Alicia.
