Re: Mrs. Charlottr Prince, aged 86
I am referring Mrs. Charlotte Prince, who has been diagnosed with mild concussion and infected right toe requires continuing of care and appropriate management following the discharge today.
On 02 December Mrs. Prince was admitted to hospital with the complaint of fall. During hospitalization noticed infected right toe, which was cleaned and dressing applied. Hence amoxicillin 500 mg was commenced. Subsequently, concussion test was satisfactory along with finger-nose-finger test and CT scan revealed satisfactory. She was experiencing urinary incontinence and mild scoliosis requiring walking stick for the mobilization. Hence ready for dicharge home.
Mrs. Prince has had hypertension and mild scoliosis which was merged with ramipril 10mg and ibuprofen whenever required. Education regardig hand hygiene to visit general practitioner.
Mrs. Prince a retired postal worker lives along , in a three bed rokm house, preferred to live independently. Reluctant to visitors and meal delivery service. She has habits of occassional consumption od whisky and smoking.
Based on the above mentioned circumstances, home visitsnby a community nurse twice a week is necessary, assissting for showering and dressing is required. Encouragement o f proper rest and change of dressing twice a week is highly recommended. Monitoring if medication complaince would be profoundly beneficial.
