Re: Ms. Albert Desouza, aged 45
I am referring to Ms. Albert Desouza, who was diagnosed with myocardial infection, requires continuity of care and followup from your estemeed services following her discharge today.
Mr. Desouza is a widow and lives alone, She leads sedentary lifestyle. she is a chronic smoker, smoke 10 cigarettes a day.
On 18 July, Mrs. Desouza was presented with chest pain raiating to shoulder, breathlessness, abdominal pain, weakness and increased sweating. Electrocardiogram reveals myocardial infractions which was managed with streptokinase a thrombolytic agent. Her cardiac triag reports shows positive result. Oxygen therapy was initiated pain reduction is noted . Head end elevation was advised. Her ultrasound reports right renal stone, referred tpo nephrological low sodium diet was advised by nephrologist.
Mr. Desouza has had hypertension and diabetics since 2010 and 2014 respectively, for which she currently takes anti-hypertensive agent amlodipine and with conservative management. Cellulites is dressing. She has an appointment with cardiologist after three weeks. Her diet will be supervised by dietician.
In view of above circumstances, regular monitoring of Ms. Desouza's diet regime, blood pressure and blood sugar 3 times per week would be highly appreciated to improve her condition.
