Dear Community Nurse,

Re: Ms. Monica Osburn, aged 69

I am referring Ms. Monica Osburn, who requires continuing care from your estmeed services following her shift to woodville on her daughter's request.

Ms. Osburn lives alone in arented house. She is moving to rented one bedroom unit in woodville near to her daughter. She has a history of excessive alcohol intake,when she has increased anxious. He has a history of hyoertensive and depression for 10 years and 2 years respectively. currently she is an antihypertensive and antidepressants. He has periodical self adminitration problems monitoring of he medication adherence is highly recommended.

Upon referral establishment of contact with medical practiotioner would be highly appreciated after her shift. Encourage her to expansion if family and social activities includes elderly citizens club, volumnary groups etc.

In view of above circumstances, monitoring of her medication adn alcohol would be extremely beneficial to improve Ms. Oshwen's condition.
