Re: Ms Mary Belly, Aged 66
I am referring ms mary bell , who was diagnosed with unstable diabetes mellitus and a small infected left foot ulcer. She requires continuing care and lifestyle modifications. following her discharge today.
Ms Bell is retired and lives alone in her own four bedroom home. She requires small pension after her retirement . Stablistment of an income generating activity is advised . After her retirement increased alcohol intake and decreased dietary quality were reported . She has poor social network . She has periodic problem with self administration of hypoglycaemic medication . Her current medications includes glimel, 5mg metformin, 850mg for her diabetes, Augmentim duo forte, 875/125 mg which have to continued until she course completion and review for it.
During Hospitalization , mrs bell's foot ulcer was managed with regular daily salime dressing changes need to be continued with regular daily changes need to be continued after her discharge . Assessment of wound healing process and wound margin were advised . Involvement in social activities and community services are recommended. Regular physical exercise programme has to initiated.
Based on view of above circumstances , regular monitoring of medication compliance , dietary regimen. Blood sugar level and exercise would be extermely beneficial to improve ms bell's condition
