Re: Mrs Silvia Rule, aged 69.


Thank you for accepting Mrs Rule, who was diagnosed with pneumonial and Algheimers disease. She requires continuity of care at your facility following her discharge today


On 10 June, Mrs Rule was presented with exacerbated symptoms of bouterial chest infection. During admission, Aloheimer's disease was evidenced by her. behavioural changes. Oxygen therapy and injection amoxicillin 500mg three times doing were prescribed. An instant discharge hor family was advised after discussion with her family.


Mrs Rule has had hypertension. She hasthe habits of occasional smoking and drinking. He parents are staying in nursing home as well Her two daughters and three sons are living near to the centre. She had recurrent- hospitalization for the same within three months


Today, her fever and other respiratory distress became reduced. However, it has to be monitored with the symptoms as well as the medication.


Based on the aforementioned considerations ongoing assistence and regular observation of Mrs Rule physical as well as mental condition would be extremely advantageous. In case you require any additional information, please do not hesitate to contact me.
