Re: Mr Alexander Jones, aged 60


Dear Nurse,


I am referring Mr Alexander Jones, who was diagnosed with cataract, for ongoing care and appropriate management at your facility, following the discharge today.


Past medical history revealed that he has had diabetes and severe headache for the last 10 years and 2 months respectively. Additionally, he is a chain smoker, lives alone in his house. Apart from that antibiotics and eye drops were commenced. Dietary supplements and fluids were given as well as the eye bandaging was performed.


Following discharge, performing household activities is not possible thing. Therefore he requires the assistance of 3 weeks. Improving the dietary quality by providing nutritious components is suggested. She needs to wear sunglasses due to UVA and UVB rays. Rigorous activities and exposure to dust are prohibited.
