Re: Mr Alfred Billy, aged 52

Dear Nurse,

I am referring Mr Alfred Billy, who underwent a surgery for nodular basal cell carcinoma, for ongoing care and appropriate management at your facility, following the discharge today.

Mr Billy a truck driver, lives with his wife, an alcoholic as well as chain smoker for the last 30 years. Apart from that his dark colour of the skin might be the reason for his cancer.

As per examination, excellent performance was noted without any complications. The biopsy of skin was performed. Tablet panadein Forte 500 mg for pain was commenced.

Following discharge, regular monitoring is highly recommended. Additionally, medication should be taken for one week. The condition of the patient is good without any pain.

If any additional information is required. Please do not hesitate to contact me.

Yours sincerely
