RE: Mrs Mandy Chang, D.O.B: 25/09/1952
I am refering Mrs Mondy Chang, who was admitted for symptoms suggestive of myocardial infarction. Therefore, she requires further management from your facility today.
Upon admission an 23/09/2020, Mrs Mandy presented with shortness of breath, chest pain and pair is right hand and shoulder, the reported to have recieved Aspirin 2 tabs at home. On assessment, vital signs were checked and pulse was slightly irregular. In addition to that, further investigations were done and was observed for 24 hours. Blood Sugar was checked, it was 160mg/dl and it was manged with metformin on 25/09/2020, vital signs were with normal limits. She was commenced on Aspirin 75mg co, clopidogrel 150mg 00, metformin 850mg OD and Atorvastatin 40mg Once
Concerning her parist medical history, Mrs Mandy is a type 2 diabetic patient Since 2018 on metformin 850mg OD. Also, she is allergic to penicillin.
Socially, Mrs chang is a widower. Her next of kin is Mellisa Garplen. She does not smoke nor does any physical exercises.
Based on the above, it would be much appreciated if you could render Mrs Chang with further management Also, she has to do exercises and also needs be educated on her current condition.
