Re: Mr Joshua Gibbon,aged 27


Mr Joshua Gibben, who has been diagnosed with boxer's fracture on the fifth metacarpel is being referred to your facility for continuing care and expedient management.


Today, Mr Gibbon has been admitted with the complaint of numbness on the left hand after being hit against a wall.Radiological examination confirmed the diagnosis;therefore, he underwent closed reduction.Hand elevation along with splint application was performed to facilitate immobility. He has been commenced on paracetamol.


Mr Gibbon,lives with his mother and wife, had fracture fibula and fracture on the right leg in 2006 and 2008 respectively.


Upon discharge, Mr Gibbon was advised regarding cast care such as to avoid insertion of objects inside the cast,not to remove the cast as well as the significance in maintaining dryness. He was recommended to report in case any pain or numbness exhibits on the fingers or hand.Physiotherapy review and hand Clinic review also has been suggested.


Pertaining to the above details, it would be profoundly beneficial if you could render on going care and appropriate management for Mr Gibbon. Please note, a follow-up appointment has been scheduled with x-rays at Fracture Nurse Practitioner's Clinic after one week.
