Dear Ms Ponsford
RE: Mr Lionel Ramamurthy, 63 years old.
I am writing regarding Mr Ramamurthy who was admitted to our hospital on the 04/02/2019 for pneumonia. Due to making excellent progress he is scheduled to be discharged today and now requires your continued monitoring and assistance.
During hospitalization, he was suffering from shortness of bresth, fever, rigors, sleeplessness, inspiratory and expiratory wheezing, persistent coughing which led to a chest and abdominal pain, and generalised body ache. He was assisted with activities of daily living. He has been seen by the dietician and physiotherapist who has given his advice and instructions to aid a move speedy and full recovery. As a consequence, he has gained 1.5kg in weight.
It would be greatly appreciated if you could administer paracetamol if needed for his chest and abdominal pain, and always keep him warm. Please monitor and encourage him to increase his fluids, eggs, fruit and vegetable intake. In addition, kindly encourage him to do deep breathing and coughing exercises and to ambulate regularly. He will need to sit up more them lying down to ensure postural drainage.
