Dear Ms Williams,

Re: Mr Yanlin Ma, DOB: 12 March 1980

Mr Yanlin Ma, who has been recuperating from cardio - thoracic surgery, requires social assistance and evaluation from your service before his discharge. He is scheduled for discharge on 9 April 2012.

Mr Ma, a Chinese student of Information technology, lives alone and has no family in Australia. However, his mother has recently visited to take care. He is overweight and smokes 5-6 cigarettes daily. Apart from that the lease on rental accommodation has expired in addition University assessment for this semester will not be completed and the visa is near to expire at the end of the semester for reducing weight and quit smoking has been noted

On 31 March 2012 represented to to royal Brisbane and women's hospital with severe chest pain on the same day he was transferred to our hospital for the surgery.

Based on the aforementioned circumstances, it would be greatly appreciated if you could arrange a new house for Mr Ma and his mother. Contacting to the university for external study resources and immigration department for visa extension would be profoundly beneficial. Liasing with smoking cessation and weight loss programs is recommended.

In case additional information is required, please do not hesitate to contact me.

Yours sincerely,

Annie Smith
