I am referring Mrs Patricia Walshman, who has suggestive features of Alzhmeirs disease associated with dementia. She requires full memory assessment and diagnosis from your facility.

Today, Mrs Welshman and her daughter visited our practice with the complaints of noos memory, decision making issues related to family and behavioural changes. On mini-memory assessment and years were noticed and she looks worried. However, her daughter requested further in-depth investigations to rule and the issue.

In turns of past medical and social history, Mrs Welshman is a widow, resides alone although children are within short distance. She has family history of alzheimer’/ms and her medical history is significant for hyperlipidemia and low Vitamin D owing to non-complaince towards webster pack off lately, she is on regular treatment.

In the light of the alone information, it would be profoundly beneficial if you could provide your valuable evaluation of Mrs Welshman’s condition for further process.

A Review has been scheduled after post-assessment in our facility.
