Dear Ms Petchell,

RE: Ms Eiden McGrath, DOB: 16/08/1965

I am writing regarding Ms McGrath, who visited our GP on 05/07/2019, for a foot ulcer (grade 2 wound) caused by a shoestring. Due to making excellent progress, she is being referred to you for further care and management.

During the consultation, she presented with a swollen and tenderfoot, and she also complained of tiredness and lethargy. Her blood sugar was elevated and she was diagnosed with type II diabetes and osteomyelitis. Her wound was dressed accordingly.

She has a medical history of hypertension and she is currently taking fortamet anti-diabetic, albactin antibiotic as well as tapcynta for pain.

Ms McGrath has been advised to continue his moist dressing for two days and reduce alcohol intake. Type 2 diabetes and its medication were explained, and she should replace tapcynta with duvolt 50mg after one week. A brochure on diabetes management was given, and roomy, well-cushioned footwear was suggested to her. She was educated to have enough bed rest and to avoid weight-bearing. The importance of a healthy diet was discussed with her. A walking stick has been provided for her by the GP.

A follow-up appointment has been organized in one week's time after the review by the podiatrist.

It would be greatly appreciated if you could provide further assessment and support for Ms McGrath.
