Re: Ms Jennifer Linda Allen, aged 83
Ms Jennifer Linda Allen, who had an angioplasty five days ago, requires further assistance and care from your facility, following her discharge tomorrow.
Ms Ellen lives alone in her own house she has little interest in meeting with anyone. She prefers meals - on - wheels. Apart from that, her current BMI is 29.5. Initially, she had angioplasty in 2012, following which aspirin and clopidogrel were commenced. Also, she has been on metoprolol, ramipril and atorvastatin for hypertension and hypercholesterolemia, respectively. She has family history of coronary heart disease.
On 11 March 2019, she was admitted with angina; therefore, a repeat angioplasty was performed. Postoperatively, fluids followed by light diet were provided. Catheter site shows no indication of complication. Anxiousness related to managing health and daily activities post - discharge have been noted.
Based on the aforementioned circumstances, small and frequent diets including high - protein, low - cholesterol and gluten - free diet as well as adequate amount of fluids would be profoundly beneficial for Ms Allen. Mild exercises needs to be continued under the physiotherapist's supervision. Lifting heavy objects is being restricted for next 12 months. Incision site, medication - compliance and danger of social isolation should be assessed by Community Health Nurse via home visits.
