Dear Nurse,
Re: Mr Alfred Billy, aged 52
Mr Billy who is recuperating from basal cell carcinoma surgery requires, ongoing care and further management following his discharge.
On 21 March 2010, he was admitted and diagnosed with skin cancer; therefore, CCB removal was done and also a skin biopsy was taken for pathological study.Postoperatively mild infection was noticed at the site of surgery along with skin breakdown probably due to incomplete excision.
Mr Billy’s condition gradually improved with good healing process and no persistent swelling was observed. Panadein forte 500mg was commenced for pain and to be continued for one more week. Presently he is fit for discharge with no complications as well as with no pain.
Mr Billy is married , works as a truck driver. He is a smoker and has habit of taking alcohol since 30 years . His past medical history is unremarkable. In case of any emergency, his wife to be contacted as she lives separately.
Based on the above mentioned details, it is necessary to monitor the site wound for any infection and to provide ongoing care and management.
If you have any queries, please do not hesitate to contact me.
Yours faithfully
