Re:Mr Lionel Ramamurthy,aged 63


I am writing to discharge Mr Ramamurthy who was hospitalized due to pneumonia,he is now ready to be discharged back to your care.

Mr Ramamurthy was admitted at new town hospital on 4 February 2019,with acute shortness of breath,wheezing,fever,persistent cough,insomnia and general body ache.during hospitalization he was initiated with walking with frame for ambulation and assisted in dressing and bathing,also physiotherapy for deep breathing and coughing exercises.

Mr Ramamurthy was progressing well,he is able to walk slow and also independently with activities of daily living.he was seen by dietician because of weakness during admission and was maintain on good nutrition,he has additional weight and need continuous diet monitoring.

Base on the above mentioned circumstances Mr Ramamurthy need to be monitored, ensure intake of oral fluids, serve paracetamol in case of abdominal and chest pain,kept warm also it would be profoundly beneficial if you continue to maintain him on sitting position to ensure postural drainage.
