Dear Director,
I am referring Ms Carpenter, who has been diagnosed with left lung resection. She requires continuing care and definitive management at your service following her discharge today.
Ms Carpenter was admitted to the intensive care unit of Flinders Medical centre on 10 May 2009.During hospitalization, she developed pneumonia, for which she was treated with ventilation and antibiotic therapy. In addition, she was provided with nutritional support, fluid management therapy as well as oxygen therapy. Currently, she has made a good prognosis.
Ms Carpenter, recent migrant from Canada, is a divorcee and has two children.
She has had breast cancer in 1988; therefore,she undergone mastectomy. Although she responded well to tamoxifen, remission occurred in 2009.Subsequently ,investigations revealed small patches in the left lung. She also has generalised anxiety disorder.
Pertaining to the above details, it would be profoundly beneficial if you could take over the care of Ms Carpenter and monitor her medications. It is essential to encourage mobility by ensuring on going Physiotherapy. Kindly arrange psychiatrist's visit to improve coping strategies, whereas meeting with her daughters also is significant in order to enhance emotional support. Please note, a follow-up appointment has been scheduled on 06 June 2009 at Flinders
Medical Centre for chemotherapy.
