Dear Sir/Madam,
Re:Mr Alfred Billy, aged 52
Mr Billy, who had undergone skin biopsy requires ongoing care and assistance from your facility following the discharge today.
On 21 March 2010, Mr Bailey was admitted to hospital with that nice to skin cancer and nodular basal-cell carcinoma. He underwent skin biopsy and sample was taken for pathological studies. Panadeine forte,500mg was administered to control pain and no complaints was observed.
Mr Billy, who lives with wife Mrs Maria Jennifer. No history of any illness and hospitalisation were reported. He consumes alcohol for the past 30 years and smokes cigarettes. He works as a truck driver. If there is any sort of emergency observed, please contact his wife as early as possible. His condition was extremely progressed during the time of discharge and reported no pain.
Based on the aforementioned circumstances, Mr Billy needs to continue medication for one more week and daily observation is very beneficial.
If any additional information is required, please do not hesitate to contact me.
Yours faithfully,
