Re: Mrs Charlotte Prince, aged 86


Mrs Prince, was diagnosed with mild concussion requires ongoing care and assistance. She also request home visit from your care, following her discharge today.


On 2 December 2018, Mrs Prince was admitted to hospital following a fall at home. Walking stick was used to mobilise due to mild scoliosis. During hospitalisation, observed her tight toe was infected. Dressing and antibiotics (amoxicillin ,500 mg)were prescribed. Monitoring and precautionary admission was advised. Her test results were satisfactory and ready to be discharged.


Mrs Prince, who is a retired postal worker lives alone. Her husband died 14 years ago, and has two children. She has a history of hypertension from past 25 years, for which she is commenced on ramipril 10mg. Also had, urinary incontinence and ibuprofen, were prescribed for scoliosis. Due to poor hygienic practices conjunctivitis and

gastroenteritis were developed. Therefore hesitate to visit general practitioner. Medication complains was observed and therefore, twice a week home visits as well as monitoring is beneficial. Encouraged adequate rest and the importance of dressing.


It on the above circumstances, assistance is required to perform daily activities. Advice regarding personal and food hygiene was provided.
