Re: Ms Sheila Cartwright, aged 69
Ms Cartwright , who is recuperating from a small infected left foot ulcer secondary to diabetes mellitus requires, ongoing care and further management on overall life style plan following her discharge today.
On 14 April 2017, she was admitted and diagnosed with the above mentioned diagnosis, for which antibiotic treatment was initiated along with aseptic wound management. Augmentin duo forte 125mg twice daily along with regular hypoglycemic medications glibenclamide 5mg daily once and metformin 800mg thrice daily should be continued.
Ms Cartwright is a bachelor, resides alone in her own house and gets very low pension . She has no friends or relatives to support her. After retirement, she started to increase alcohol intake and finds difficulty with medication compliance and dietary regimen.
In view of her condition, it is very necessary to monitor medication compliance and there is a review after completion of antibiotic course. Regular daily dressing at the site of wound as well as monitoring for any signs of infection is required.
More attention on monitoring blood sugar levels, dietary regimen and alcohol intake is necessary, along with regular exercise program. It would also be very beneficial to Ms Cartwright to get involved in some income producing activities; therefore, review from community social service worker is needed.
Based on the above mentioned details, it would be greatly appreciated if you could follow up with her medical and social needs.
