Dear Dr Dane,
Re: Mr Timathy Patterson,aged 69
I am writing to update the current health status of Mr Timathy Patterson who requires cardiac assessment and management at your facility. He is suspected to have orthostatic hypertension.
Mr Patterson was admitted to hospital on 01 July 2019 with deep lacerations on the lower left arm following a cut. He was treated with prophylactic antibiotics. The wound was managed with sutures and daily dressing. His sitting and supine blood pressure were 140/90 mmHg and 180/90 mmHg respectively. He was advised to visit after discharge on 03 July 2019.
During his followup visits Mr Patterson's wound has healed well with unremarkable infection. However, blood pressure is continued to be elevated which is about 190/80 mmHg at sitting and 196/26 mmHg at supine position.
Based on the above mentioned circumstances, full cardiac assessment and treatment for elevated hypertension would be profoundly beneficial.
If you have any queries regarding Mr Patterson, please do not hesitate to contact me.
Yours sincerely,
Amber Diva
