Re: Ms Patricia Styles, aged 62
I am referring Ms Styles, who is suspected to have a complication of pericarditis. She requires your urgent evaluation and further management.
On 30 August 2019, during the home visit, Ms Styles has been reported with chest pain, difficulty in breathing, and associated ailments. After the assessment, her vital parameters were abnormal. On 23 August 2019, she was admitted to the hospital with chest discomfort, which was exacerbated by breathing, and showed an elevated heart rate. After the examination, one echocardiogram was performed, which revealed the pericarditis; therefore, intravenous fluid and tablet ibuprofen 600 mg every 8 hourly were commenced. Subsequently, she was discharged with some home care assistance.
Ms Styles, who is a pensioner, lives on his own accommodation. She has had hypertension and diabetes mellitus; therefore, she is on tablet quinapril 40 mg, tablet metformin 500 mg twice daily, and tablet gliclazide 30 mg daily.
Pertaining to the above-mentioned circumstances, kindly provide the immediate assessment and treatment for him. All the previous test test results and medication lists are attached with this letter for your reference.
