Dear Nurse
Re: Ms Monica Osburn, aged 69
I am writing to request a homevisit for Ms Monica Osburn, who has been diagnosed with hypertension and depression. She requires regular monitoring and continuing care.
Ms Osburn, has had hypertension since 10 years and depression since 2 years forwhich, she is on antihypertensives and antidepressants respectively; however she has been facing problems with medication adherence particularly in self administration.
Ms Osburn, is a aged pensioner, resides alone in a rented house in Newport. Although, she has been shift to rented house in Woodville, she will not feel lonely because her daughter lives near to her. In addition she has the habit of severe alcohol consumption while in anxious situations.
It would be beneficial to Ms Osburn, if you could monitor the medication compliance and diet regimen.Apart from these provide health education regarding the consequences of alcohol intake. Your instructions is essential to initiate her consultation with medical practitioner after her move. Kindly arrange to join the elderly citizens clubs and voluntary groups to improve her friendly relationships with family and society.
Based on the aforementioned circumstances, your assistance is profoundly beneficial. In case any additional information requires, please do not hesitate to contact me.
Yours faithfully,
Registered Nurse
