Dear Ms Smith,
Re: Mr Abdullah Gahtani, DOB: 05 June 1968
I am referring Mr Abdullah Gahtani, who has been recuperating from coronary artery bypass graft surgery. He requires continuing care and definitive management from your service following his discharge today.
On 07 August 2018, Mr Gahtani was admitted with the complaints of severe chest pain,along with pain in the left arm and jaw. Chest x- ray and echocardiogram revealed myocardial infarction;therefore, he underwent surgery. He has been treated with streptokinase,anticoagulants,anticholenergics,Karvey 150mg and Aspirin 81 mg daily. Physiotherapy has been commenced which needs to be continued after the discharge.
Mr Gahtani, a refugee from Morocco to Australia, works as a store keeper. He is married, lives in a rented flat and has no proficiency in English. Specifically,he is obese,and has no family doctor. He is a smoker, for which he was advised about the significance of smoking cessation.
Upon discharge, it would be profoundly beneficial if you could instruct Mr Gahtani to collect medications from the pharmacy and adhere to medication regimen.Kindly arrange a dietitian's advice whereas, monitor diet,heart rate and blood pressure. Please provide access to a social worker to facilitate registration with local general practitioner for the entire family. Kindly note, a follow-up appointment has been scheduled with the cardiologist on 25 August 2018 at 2pm.
