Re: Ms Bethany Tailor, aged 35
Ms Tailor was self admitted on 01 March 2018 with decompensated schizophrenia probably owing to non-compliance with medications. She requires follow- up care and is being discharged back into apartment today.
On admission, visual hallucinations, auditory command and delusions were experienced. As a result of this, she found to be agitated and agressive; therefore, antipsychotic medicine (resperidone) was commenced which needs to be continued.
On subsequent days, auditory or visual hallucinations has been ceased. In addition to that she has less disordered thinking and capable to reduce delusion process and also able to focus on activities of daily living. On assessment, excellent progress has been observed.
Ms Tailor has had hypertension secondary to fibromuscular dysplasia and has hypothyrodism, for which she takes levothyroxin 88mcg daily. Please note , she has a history of polysubstance abuse especially cocaine and alcohol.
In view of the above, evaluation is necessary for objectieve signs of psychosis. Redirecting from delusions and ensuring medication compliance are suggested. Apart from this, maintaining behavioural control as well as providing therapy if possible. Please note that resperidone 1milligram two times daily if required for agitataion.
