Re:Mr Abdullah Gahtani

I am requesting home visits for Mr Abdullah Gahtani who requires further care and management.He underwent a coronary artery bypass graft procedure and is being discharged today.
Mr Gahtani was admitted to hospital on 07August 2018 with the complaints of chest,jaw and left arm pain.A chest x-ray and an electrocardiogram confirmed myocardial infarction.As a result surgery was performed for him.During hospitalization,streptokinase,karvea 150mg and aspirin 81mg daily as well as physiotherapy were commenced.
Mr Gahtani is a refugee from Moraco and he lives in a rental flat.He has language barrier; however,he works as a store keeper.He is a regular smoker and has body mass index above the recommended level.
Upon discharge,Mr Gahtani necessitates the importance of regular medication compliance.It is essential to monitor his vital signs and cope up with new dietary regimens.Kindly encourage him to avoid smoking, follow exercise plan and to lead a healthy lifestyle.Please arrange a dietician, social worker and local gp.Offnote ,he has an appointment with a cardiologist in outpatient department on 25 August 2018 at 2pm.
