Dear Mrs Penny,
Re: Ms. Shella Cartwright, aged 69
I am referring Ms. Cartwright who has been diagnosed with the unstable diabetes melitus and a small infected left foot ulcer. He requires ongoing support at your facility.
On 14 April 2017, Ms Cartwrigt was admitted to the hospital with an ulcerated left foot. After the assessment, the diagnosis has been confirmed. Consequently, tablet glibenclamide 5mg daily, tablet metformin 800 mg thrice daily, and tablet Augmentin Duo 125 mg twice daily have been commenced. Daily saline dressing, monitoring of wound margin, and observation for complications as well as a review of the healing process have been done.
Ms. Cartwright, who is a pensioner, lives alone at his own house. He has had non- insulin diabetes melitus for past 9 years. He consumes alcohol excessively and poor dietary intake since his retirement. Problems associated with the self administration of the hypoglycemic medication has been reported.
In view of the above mentioned circumstances kindly take over the case of Ms. Cartwright and provide continuing care. Observation of medication compliances, monitoring of the blood sugar level, alcohol consumption, and the assessment of the dietry intake as well as the encouragement of the moderate exercise program would be profoundly benefical. Encouragement for social activity by a community social worker is required.
