Re: Ms Sheila Cartwright, aged 69
Ms Sheila Cartwright, who is recuperating from left foot ulcer associated with unstable diabetes mellitus, requires ongoing supervision and assistance following her discharge today.
On 14 April 2017, she was admitted with small infected ulcer over left foot. In addition to the regular medication Augmentin Duo Forte 125 mg twice daily was prescribed. Saline dressing of the wound along with monitoring for signs of infection were provided. She was reviewed for medication and healing process.
Ms Cartwright lives alone. She retired at 67 and receives a small pension. She has no relatives or close friends and show little interest in outing. Non- insulin - dependent diabetes mellitus a was diagnosed nine years ago, Glimel 5 mg daily and Diabex 800 mg thrice daily were commenced. Increased alcohol consumption, reduced quality diet and frequent missed doses of medication were noted.
Based on the aforementioned circumstances, assessment for alcohol consumption, diet, blood sugar levels and medication adherence would be profoundly beneficial. Education regarding exercise regime, income producing methods and involvement in social activities needs to be provided with the aid of a social worker.
