Re:Ms Patricia Cooper, aged 67
I am referring Ms Patricia Cooper, who had an episode of choking, requires an expeditious evaluation and definite management of her swallowing function and nutritional status from your service owing to high risk of aspiration.
Today, while having breakfast, Ms Cooper has choked and consequently, exhibited distress,struggled with both hands and coughed. Eventually, the choked piece was removed and she got stabilised after two hours.
Ms Cooper, a new resident of Memory Care Clinic, is single and disinterested in socialization. She has had type 2 diabetes mellitus since 2013 and is on non- compliant diabetic low fat fiber rich diet. Her body weight is above the recommended level (BMI:30).She had a weight gain of 8 kgs over the past five months and reported increased appetite as well as demands extra servings of food. She has no teeth for which she is using dentures.
Ms Cooper has had ischemic heart disease and stroke since 2014 and 2016 respectively. She also has severe dementia since 2015;therefore, she displays confusion, disorientation and poor judgement.
Pertaining to the above details, it would be appreciated if you could provide emergency assistance for Ms Cooper in order to avoid further complications.
