Re: Ms Juanita Ferreira , DOB : 13 April , 1960
I am writing to refer Ms Ferreira who has been suffering from grade four haemorrhoids and is scheduled for haemorrhoidectomy tomorrow.
On admission , she presented with the complaint of rectal bleeding with burning pain. During physical examination, slightly tender and prolapse was evidenced. In addition, she also reported regarding perianal itching , constipation and hard stools. Therefore, pathology was ordered and results are awaited .
In nursing management, her vitals were normal . She was administered intravenous fluids and an enema was performed. Apart from this, pre - procedure assessment, post surgery management , dosage of anethesia and hospital policies were explained to her. She was also informed regarding literature of pain and procedure.
It would be greatly appreciated if you could take further care . Please note , she needs to be on nil per mouth 6 hours prior surgery. She has had type 2 diabetes mellitus and inflamed bowel syndrome. She is allergic to penicillin and aspirin.
